The RayOne Galaxy spiral intraocular lens has generated considerable interest among patients considering cataract surgery or refractive lens exchange.<\/p>\n

But choosing a premium lens should never begin with the question:<\/p>\n

\u201cWhich is the newest lens?\u201d<\/p>\n

It should begin with:<\/p>\n

\u201cWhat does my eye need, and how do I want to use my vision?\u201d<\/h2>\n

Mr Samer Hamada, Consultant Ophthalmic Surgeon and founder of Eye Clinic London, was among the early surgeons to implant RayOne Galaxy and was recognised by Rayner as an early Galaxy implanter.<\/p>\n

Here he answers some of the most important questions patients should consider before deciding whether Galaxy is right for them.<\/p>\n

What exactly is the RayOne Galaxy lens?<\/h2>\n

RayOne Galaxy is a full-range-of-vision intraocular lens made by Rayner.<\/p>\n

It replaces the natural lens of the eye during cataract surgery or refractive lens exchange and is designed to provide useful vision at distance, intermediate and near.<\/p>\n

Its distinguishing feature is a proprietary spiral refractive optic rather than the conventional diffractive ring structure used by many trifocal lenses. Rayner developed the design using an AI-based optimisation process.<\/p>\n

Is Galaxy a multifocal lens?<\/h1>\n

In everyday language, patients often call Galaxy a multifocal because it aims to provide vision at several distances.<\/p>\n

Optically, however, it is different from a conventional diffractive multifocal or trifocal lens.<\/p>\n

Galaxy uses a non-diffractive spiral structure intended to generate a continuous range of focus.<\/p>\n

For this reason, we generally describe it as a full-range-of-vision spiral IOL.<\/p>\n

Who may be a good candidate for Galaxy?<\/h1>\n

A potentially good candidate is someone who:<\/p>\n

    \n
  • has cataract or is considering refractive lens exchange;<\/li>\n
  • has otherwise healthy eyes;<\/li>\n
  • would like to reduce dependence on glasses;<\/li>\n
  • values distance, computer and near vision;<\/li>\n
  • has realistic expectations about premium lens surgery;<\/li>\n
  • understands that some visual adaptation may be necessary.<\/li>\n<\/ul>\n

    However, suitability cannot be determined from age or glasses prescription alone.<\/p>\n

    The cornea, tear film, macula, optic nerve, pupil and previous surgical history all matter.<\/p>\n<\/div><\/div><\/div><\/div><\/div>

    <\/span><\/div><\/div><\/div><\/div><\/div>

    Who may not be suitable?<\/h2>\n

    There is no single exclusion list that applies to every patient, but particular caution may be necessary in patients with significant:<\/p>\n

      \n
    • corneal irregularity;<\/li>\n
    • uncontrolled ocular surface disease;<\/li>\n
    • macular disease;<\/li>\n
    • optic nerve disease;<\/li>\n
    • advanced glaucoma;<\/li>\n
    • substantial higher-order aberrations;<\/li>\n
    • unrealistic expectations regarding completely perfect vision at every distance.<\/li>\n<\/ul>\n

      Sometimes the correct decision is to treat an underlying eye problem first.<\/p>\n

      Sometimes we choose another premium lens.<\/p>\n

      And sometimes the best optical choice is a monofocal lens.<\/p>\n

      Does dry eye matter before Galaxy surgery?<\/h2>\n

      Very much so.<\/p>\n

      The tear film is the first refracting surface of the eye.<\/p>\n

      If it is unstable, the patient’s vision can fluctuate and the measurements used to calculate the intraocular lens may also become less reliable.<\/p>\n

      Mr Hamada has extensive expertise in ocular surface disease, including dry eye, meibomian gland dysfunction and related eyelid problems. At Eye Clinic London, the ocular surface is assessed carefully before premium lens surgery, and we routinely stabilise it before proceeding with surgery.<\/p>\n

      Depending on the underlying problem, treatment may include lubricants, management of meibomian gland dysfunction, eyelid treatment or other targeted therapies. Measurements may be repeated once the ocular surface is stable.<\/p>\n

      Ocular surface care does not necessarily end after surgery. Some patients require continued treatment during recovery, and we often use treatments such as intense pulsed light therapy, or IPL, after surgery when appropriate to support the meibomian glands, tear-film stability and visual quality.<\/p>\n

      A premium lens cannot compensate for a poor optical surface.<\/p>\n

      Can I have Galaxy if I have astigmatism?<\/h2>\n<\/div><\/div><\/div><\/div><\/div>
      <\/span><\/div><\/div><\/div><\/div><\/div>

      Potentially, yes.<\/p>\n

      Rayner produces RayOne Galaxy Toric for patients with clinically significant corneal astigmatism.<\/p>\n

      The important issue is establishing how much of the astigmatism comes from the cornea and determining the appropriate toric power and axis.<\/p>\n

      Accurate tomography and biometry are particularly important.<\/p>\n

      Can I have Galaxy after LASIK or other laser eye surgery?<\/h2>\n

      Possibly.<\/p>\n

      Previous LASIK, PRK or other corneal refractive surgery does not automatically prevent a patient from having Galaxy.<\/p>\n

      But these eyes deserve much more detailed assessment.<\/p>\n

      Laser eye surgery changes the shape and optical characteristics of the cornea. It can also make intraocular lens calculations more challenging.<\/p>\n

      Mr Hamada therefore considers corneal tomography, aberrations, previous prescription where available, ocular surface stability and the patient’s individual visual requirements before recommending a premium lens after previous laser surgery.<\/p>\n

      In some cases Galaxy may be appropriate.<\/p>\n

      In others another optical strategy may produce more predictable vision.<\/p>\n

      Does pupil size matter?<\/h2>\n

      Pupil behaviour is one of several factors considered when selecting a premium intraocular lens.<\/p>\n

      The pupil changes size depending on lighting conditions, and this alters the portion of the lens optic through which the patient is seeing.<\/p>\n

      Rather than relying on a single pupil measurement, we consider the pupil together with corneal optics, angle kappa\/chord measurements, higher-order aberrations and the patient’s visual priorities.<\/p>\n

      It should therefore form part of the complete assessment rather than being treated as a pass-or-fail number.<\/p>\n

      Will I see halos around lights?<\/h2>\n<\/div><\/div><\/div><\/div><\/div>
      <\/span><\/div><\/div><\/div><\/div><\/div>

      You may.<\/p>\n

      It is important that patients receive a realistic answer to this question.<\/p>\n

      Galaxy’s spiral optic was designed partly to minimise dysphotopsia, and the first peer-reviewed multicentre study reported that more than 95% of halo and glare symptoms were minor, with no severe symptoms reported in that study population.<\/p>\n

      However, statistics cannot predict exactly what one individual patient will see.<\/p>\n

      Halos, glare, starbursts or shimmering can occur after any presbyopia-correcting intraocular lens.<\/p>\n

      They often improve as the eye heals and the brain neuroadapts, but this process varies between individuals.<\/p>\n

      A stable ocular surface is also important because dryness and tear-film irregularity can contribute to fluctuating vision, glare and visual discomfort. This is one reason we assess and treat the ocular surface both before surgery and, when necessary, during the postoperative period.<\/p>\n

      Is Galaxy good for night driving?<\/h2>\n<\/div><\/div><\/div><\/div><\/div>
      <\/span><\/div><\/div><\/div><\/div><\/div>
      <\/p>\n

      Many Galaxy patients are able to drive at night, but anyone for whom night driving is critically important should discuss this specifically before surgery.<\/p>\n

      We consider:<\/p>\n

        \n
      • existing night-vision symptoms;<\/li>\n
      • pupil size in dim conditions;<\/li>\n
      • corneal aberrations;<\/li>\n
      • ocular surface quality;<\/li>\n
      • retinal health;<\/li>\n
      • the patient’s tolerance of halos and glare.<\/li>\n<\/ul>\n

        For some patients, maximising the range of vision and reducing dependence on reading glasses is the priority.<\/p>\n

        For another patient, achieving the cleanest possible night-time optical quality may matter more.<\/p>\n

        The correct lens may therefore be different.<\/p>\n

        Will I be completely glasses-free?<\/h2>\n

        Many patients choose Galaxy because they would like to reduce their dependence on spectacles.<\/p>\n

        Published clinical data demonstrate a broad functional range from distance through intermediate and near vision.<\/p>\n

        But we do not promise any patient that they will never need glasses again.<\/p>\n

        Small print, poor lighting, prolonged reading and specific occupational tasks may still be easier with a weak pair of spectacles.<\/p>\n

        Our objective is functional spectacle independence, not an unrealistic promise that glasses can never be required.<\/p>\n

        Can Galaxy be used for refractive lens exchange even if I do not have cataracts?<\/h2>\n

        Yes, in appropriately selected patients.<\/p>\n

        Refractive lens exchange uses essentially the same lens-replacement principle as cataract surgery, but it is performed primarily to correct refractive error and presbyopia rather than because the natural lens is already significantly cloudy.<\/p>\n

        Because these patients often begin with relatively good corrected vision, expectations are particularly high.<\/p>\n

        That makes careful counselling and patient selection essential.<\/p>\n

        How quickly does vision recover?<\/h2>\n

        Many patients notice a significant change in their vision soon after surgery, but final visual quality is not judged on the first day.<\/p>\n

        The eye has to recover from surgery, the ocular surface has to stabilise and the brain must adapt to a new optical system.<\/p>\n

        For patients with dry eye or meibomian gland dysfunction, ocular surface treatment may be continued or adjusted after surgery. In selected cases, treatments such as IPL may be used postoperatively to support tear-film quality and comfort.<\/p>\n

        Vision can continue to refine over the following weeks.<\/p>\n

        Individual recovery varies.<\/p>\n

        Should I have Galaxy in both eyes?<\/h2>\n

        Full-range premium lenses such as Galaxy are commonly used bilaterally because the two eyes work together to create binocular visual performance.<\/p>\n

        The published Galaxy clinical study assessed patients following bilateral implantation and demonstrated further improvement binocularly compared with monocular measurements.<\/p>\n

        However, every surgical plan is individualised.<\/p>\n

        Previous surgery, asymmetric eye health or an existing lens in the other eye can change the strategy.<\/p>\n

        Is Galaxy better than PanOptix, Odyssey, Vivity, PureSee or other premium lenses?<\/h2>\n

        There is no responsible universal answer.<\/p>\n

        Those lenses use different optical technologies and are designed to achieve different balances between range of vision, quality of vision and photic phenomena.<\/p>\n

        Galaxy has promising published clinical evidence, including comparison with a diffractive trifocal in its first peer-reviewed study.<\/p>\n

        That does not prove that Galaxy is the best lens for every patient or superior to every premium lens currently available.<\/p>\n

        At Eye Clinic London we have access to different lens technologies precisely because we do not believe that one lens fits every eye.<\/p>\n

        How important is the surgeon when choosing Galaxy?<\/h2>\n

        Very important \u2014 but perhaps not for the reason patients initially imagine.<\/p>\n

        Modern cataract surgery is highly sophisticated and premium intraocular lenses are manufactured to extremely precise tolerances.<\/p>\n

        The difficult part is often not placing the lens into the eye.<\/p>\n

        It is deciding:<\/p>\n

        which lens should be placed into which eye, at what power and with what refractive target.<\/p>\n

        That decision depends on diagnostics, surgical experience, understanding of corneal and ocular surface optics, knowledge of different lens technologies and a detailed discussion with the patient.<\/p>\n

        A surgeon who offers several premium technologies can choose between them.<\/p>\n

        A surgeon with extensive expertise in ocular surface disease can recognise when the tear film, meibomian glands or eyelids need treatment before surgery and can continue managing these factors afterwards, including with treatments such as IPL when appropriate.<\/p>\n

        And an experienced refractive cataract surgeon understands that a technically successful operation is only one component of a successful visual outcome.<\/p>\n

        The exact investigations depend on the patient, but a premium lens assessment may include:<\/p>\n

        • detailed refraction;<\/li>\n
        • slit-lamp examination;<\/li>\n
        • corneal topography or tomography;<\/li>\n
        • biometry for intraocular lens calculation;<\/li>\n
        • assessment of corneal astigmatism;<\/li>\n
        • ocular surface and tear-film assessment;<\/li>\n
        • macular OCT;<\/li>\n
        • optic nerve assessment;<\/li>\n
        • pupil evaluation;<\/li>\n
        • review of previous refractive or ocular surgery.<\/li>\n<\/ul>\n

          Additional investigations may be necessary in more complex eyes.<\/p>\n

          This diagnostic stage is one of the most important parts of the entire process.<\/p>\n

          If ocular surface disease is identified, treatment may be recommended before measurements are finalised. This can include management of dry eye and meibomian gland dysfunction, with further treatment after surgery if required to maintain a stable optical surface.<\/p>\n

          What is the single most important question to ask before choosing Galaxy?<\/h2>\n

          Not:<\/p>\n

          \u201cHow new is the lens?\u201d<\/p>\n

          And not:<\/p>\n

          \u201cHow many people are glasses-free?\u201d<\/p>\n

          The question should be:<\/p>\n

          \u201cWhy is this lens the right lens for my particular eyes?\u201d<\/p>\n

          Your surgeon should be able to answer that clearly.<\/p>\n

          Galaxy is an exciting optical technology and, in appropriately selected patients, it can provide an impressive range of functional vision.<\/p>\n

          But the best results in premium cataract and refractive lens surgery come from combining good technology, good diagnostics, good ocular surface management, good surgery and good patient selection.<\/p>\n
