that patients are no longer simply choosing to have a cataract removed.<\/p>\n<p>Increasingly, they are choosing how they would like to see afterwards.<\/p>\n<p>A patient researching premium cataract surgery today may encounter terms including monofocal, enhanced monofocal, EDOF, trifocal, multifocal, toric \u2014 and now the RayOne Galaxy spiral lens.<\/p>\n<p>So where does Galaxy fit, and could it offer an advantage over a traditional multifocal, trifocal or EDOF lens?<\/p>\n<p>Mr Samer Hamada, Consultant Ophthalmic Surgeon at Eye Clinic London, explains why the answer depends on the patient \u2014 and why the reduced experience of halos and glare with Galaxy is often one of its most important advantages in clinical practice.<\/p>\n<h2 data-fontsize=\"35\" style=\"--fontSize: 35;\" data-lineheight=\"normal\" class=\"fusion-responsive-typography-calculated\">First: there is no single \u201cbest cataract lens\u201d<\/h2>\n<p>This is important.<\/p>\n<p>There are excellent lenses in several different categories, but every optical design involves priorities and compromises.<\/p>\n<p>The objective is therefore not to identify the lens with the longest list of features.<\/p>\n<p>It is to identify the lens whose optical characteristics best match:<\/p>\n<ul>\n<li>the health of your eyes;<\/li>\n<li>your visual priorities;<\/li>\n<li>the distances at which you spend most of your day;<\/li>\n<li>your tolerance of optical phenomena;<\/li>\n<li>your occupation and hobbies;<\/li>\n<li>your expectations regarding glasses.<\/li>\n<\/ul>\n<p>At Eye Clinic London, this is the basis on which Mr Hamada approaches premium lens selection.<\/p>\n<p>However, in our experience, one issue repeatedly influences patients\u2019 decisions: how much they want to reduce the risk of halos and glare while still achieving a broad range of vision.<\/p>\n<p>For many patients, this becomes one of the most persuasive advantages of Galaxy.<\/p>\n<h2 data-fontsize=\"35\" style=\"--fontSize: 35;\" data-lineheight=\"normal\" class=\"fusion-responsive-typography-calculated\">Monofocal lenses<\/h2>\n<p>A conventional monofocal intraocular lens provides one principal focal point.<\/p>\n<p>Most commonly, both eyes are targeted for good distance vision.<\/p>\n<h3>Advantages<\/h3>\n<p>Monofocal lenses generally provide excellent optical quality and a relatively low risk of halos and glare caused by multifocal optics.<\/p>\n<h3>Limitations<\/h3>\n<p>If both eyes are focused for distance, patients will usually require reading glasses and may require glasses for intermediate activities.<\/p>\n<p>Monovision can sometimes be used, where one eye is targeted slightly closer than the other, but this is not appropriate for everyone.<\/p>\n<h2 data-fontsize=\"35\" style=\"--fontSize: 35;\" data-lineheight=\"normal\" class=\"fusion-responsive-typography-calculated\">Enhanced monofocal lenses<\/h2>\n<p>Enhanced monofocal lenses attempt to extend the range of useful vision beyond that of a conventional monofocal while retaining optical characteristics closer to a monofocal lens.<\/p>\n<p>They can be particularly useful for patients who prioritise distance and intermediate vision but are willing to use glasses for closer reading.<\/p>\n<p>For example, Rayner&#8217;s RayOne EMV is designed to provide an increased range of focus and can also be used with a modest monovision strategy.<\/p>\n<p>The trade-off is that they generally do not aim to provide the same level of near spectacle independence as a full-range premium lens.<\/p>\n<h1>EDOF lenses<\/h1>\n<p>EDOF stands for extended depth of focus.<\/p>\n<p>Rather than creating several sharply separated focal points, these lenses attempt to elongate the focal range.<\/p>\n<p>They are often particularly strong at distance and intermediate vision.<\/p>\n<p>For some patients this creates a very natural functional range, particularly for driving, computer work, cooking, sport and everyday activities.<\/p>\n<p>Near vision varies considerably between EDOF designs and patients, and reading glasses may still be required for small print.<\/p>\n<p>EDOF lenses may also appeal to patients who are especially concerned about night-time visual symptoms, although the exact experience depends on the individual lens design, the patient\u2019s eyes and their visual expectations.<\/p>\n<h1>Traditional trifocal and multifocal lenses<\/h1>\n<p>Diffractive trifocal lenses have been one of the principal technologies used to provide distance, intermediate and near vision.<\/p>\n<p>Their optical structures distribute light into different focal points.<\/p>\n<p>For carefully selected patients, they can provide a very high level of spectacle independence.<\/p>\n<p>However, because light is being distributed between different focal ranges, some patients experience halos, glare, starbursts or reduced contrast, especially in low-light situations.<\/p>\n<p>Many patients adapt well, but not everybody responds to multifocal optics in the same way.<\/p>\n<p>This is particularly relevant for patients who drive at night, work in low-light environments or are highly sensitive to visual disturbances. Rayner reports that the spiral design was developed using an artificial-intelligence-based optimisation process.<\/p>\n<p>Instead of generating three distinct focal peaks, the aim is to produce a smoother and more continuous range of focus.<\/p>\n<p>This places Galaxy in the full-range-of-vision category, although patients may understandably refer to it as a multifocal lens because its objective is to reduce dependence on glasses across multiple distances.<\/p>\n<p>In our clinic, one of the most important practical observations is that patients receiving Galaxy commonly report relatively little difficulty with halos and glare, particularly during activities such as night driving. This is based on our patients\u2019 feedback and our own clinical experience with the lens. It is not a guarantee that every patient will experience completely symptom-free vision, but it is frequently one of the strongest advantages we discuss with suitable patients.<\/p>\n<h2>Galaxy versus a trifocal lens<\/h2>\n<p>There are two particularly interesting differences.<\/p>\n<h3>1. How the range of vision is created<\/h3>\n<p>Traditional diffractive trifocals create designated focal points.<\/p>\n<p>Galaxy uses its spiral refractive optic to create a more continuous progression through the focal range.<\/p>\n<h3>2. Photic phenomena<\/h3>\n<p>Halos and glare remain one of the principal concerns patients have when considering a full-range lens.<\/p>\n<p>The first peer-reviewed Galaxy study included preclinical comparison with Rayner&#8217;s diffractive trifocal IOL. It found a comparable range of focus in the testing conditions but significantly less halo and glare with Galaxy in that preclinical comparison.<\/p>\n<p>The subsequent multicentre clinical component also reported generally minor halo and glare symptoms in more than 95% of patients, with no severe cases reported within that study population.<\/p>\n<p>These findings are consistent with what we often see clinically: for many appropriately selected patients, Galaxy provides a broad range of vision with fewer troublesome halos and glares than they may associate with traditional multifocal or trifocal lenses.<\/p>\n<p>Our patients\u2019 experiences are particularly encouraging in relation to night driving, headlights and street lighting. Many report that they can perform these activities comfortably without the level of distracting halos or glare they had feared. This reflects our clinical experience and patient feedback, rather than a promise that every patient will have the same result.<\/p>\n<p>This is promising evidence, but it should not be interpreted as meaning that Galaxy can never cause halos or glare.<\/p>\n<p>No presbyopia-correcting lens can make that guarantee.<\/p>\n<p>The important point is that the balance between range of vision and photic phenomena appears particularly attractive for many patients.<\/p>\n<p>Why fewer halos and glares can matter so much<\/p>\n<p>Patients often focus initially on whether they will be able to read without glasses.<\/p>\n<p>That is understandable, but it is only one part of the visual experience.<\/p>\n<p>A patient may also want to:<\/p>\n<ol><\/ol>\n<ul>\n<li>see streetlights and headlights without distracting rings;<\/li>\n<li>use a phone in dim lighting;<\/li>\n<li>watch television without visual disturbances;<\/li>\n<li>move between bright and dark environments comfortably;<\/li>\n<li>maintain good contrast and visual quality in everyday conditions.<\/li>\n<\/ul>\n<p>For many patients, the possibility of reading without glasses is attractive, but not if it comes with persistent halos or glare that interfere with daily life.<\/p>\n<p>This is why the reduced experience of halos and glare is often the best selling point of Galaxy in our clinic. Based on our patients\u2019 feedback and our experience with the lens, it offers the possibility of combining a broad range of vision with a more comfortable visual experience than some patients may expect from a traditional diffractive multifocal or trifocal lens.<\/p>\n<p>For patients who drive at night, this can be especially important. The ability to see headlights, traffic lights and street lighting without distracting rings or glare may have a greater effect on satisfaction than near vision alone.<\/p>\n<h2>Galaxy versus EDOF<\/h2>\n<p>This comparison is slightly different.<\/p>\n<p>An EDOF lens is often selected for patients who prioritise excellent distance and intermediate vision and are prepared to accept some dependence on reading glasses.<\/p>\n<p>Galaxy aims to extend useful vision further into the near range while preserving a continuous focal profile.<\/p>\n<p>In simple terms:<\/p>\n<ul>\n<li>EDOF: often distance + intermediate focused.<\/li>\n<li>Galaxy: designed to provide distance + intermediate + near.<\/li>\n<\/ul>\n<p>That sounds as though Galaxy should automatically be the better choice, but this is exactly where individual selection becomes important.<\/p>\n<p>A patient who drives frequently at night and has relatively modest requirements for near spectacle independence may deliberately prefer a lower-range optic.<\/p>\n<p>Another patient may consider freedom from reading glasses one of their highest priorities and may find Galaxy\u2019s combination of near vision and relatively low levels of halos and glare particularly appealing.<\/p>\n<p>Neither patient is wrong.<\/p>\n<p>Their requirements are simply different.<\/p>\n<h2>What about Galaxy and near vision?<\/h2>\n<\/div><\/div><\/div><\/div><\/div><div class=\"fusion-fullwidth fullwidth-box fusion-builder-row-4 fusion-flex-container has-pattern-background has-mask-background strong near and intermediate performance, with binocular visual acuity of 20\/32 or better maintained through a broad defocus range extending to approximately 35 cm.<\/p>\n<p>That is encouraging for everyday tasks such as smartphones, menus and reading.<\/p>\n<p>However, no surgeon should promise that a premium lens will make glasses unnecessary for absolutely every task.<\/p>\n<p>Tiny print, prolonged reading and poor lighting remain more demanding visual situations.<\/p>\n<p>The advantage of Galaxy is therefore not simply that it aims to provide near vision. It is that it seeks to provide near, intermediate and distance vision while maintaining a smoother optical profile and, for many patients, a lower burden of halos and glare.<\/p>\n<h2>Does Galaxy mean fewer halos than every trifocal?<\/h2>\n<p>We would be cautious about making that claim.<\/p>\n<p>The peer-reviewed study provides encouraging comparison against the specific diffractive trifocal lens tested, but that does not mean Galaxy has been proven superior to every premium intraocular lens currently available.<\/p>\n<p>Different manufacturers use different optical principles.<\/p>\n<p>Patient populations also differ, and long-term independent comparative evidence continues to evolve.<\/p>\n<p>At Eye Clinic London, we therefore use clinical evidence as part of the decision rather than turning one study into a marketing slogan.<\/p>\n<p>At the same time, our own clinical experience is important. We regularly see that the reduced experience of halos and glare is one of the features patients value most after Galaxy implantation. Our patients often describe night driving as more comfortable than they expected, with fewer distracting halos and glares around headlights and streetlights. It is frequently the factor that makes the lens particularly attractive to patients who want a broad range of vision but are worried about the night-time symptoms sometimes associated with multifocal optics.<\/p>\n<p>These observations are based on our patients\u2019 experiences and our clinical experience with the lens. They should be understood as an important practical pattern, not as a guarantee of an identical outcome for every patient.<\/p>\n<h2>What if I have astigmatism?<\/h2>\n<p>Astigmatism is extremely important when choosing any premium lens.<\/p>\n<p>Galaxy is available as RayOne Galaxy Toric, allowing suitable patients with corneal astigmatism to combine astigmatic correction with the Galaxy spiral optic.<\/p>\n<p>Accurate corneal measurements, calculation and rotational positioning of a toric lens are essential because residual astigmatism can reduce the quality of vision from any premium IOL and may also make halos or glare more noticeable.<\/p>\n<h2>What if I already have dry eye?<\/h2>\n<p>This is an often overlooked question.<\/p>\n<p>The tear film forms the first optical surface of the eye. Significant dry eye or meibomian gland dysfunction can affect both the accuracy of pre-operative measurements and the quality of vision after surgery.<\/p>\n<p>For that reason, at Eye Clinic London the ocular surface is assessed carefully and, where necessary, treated before final lens measurements and surgery.<\/p>\n<p>Putting an excellent premium lens into an eye with an unstable optical surface does not automatically produce excellent vision.<\/p>\n<p>Dry eye can also make visual symptoms such as glare, fluctuating vision and light sensitivity more noticeable, so treating the ocular broad range of vision with fewer troublesome halos and glares than they may experience with some traditional multifocal or trifocal designs, particularly when driving at night.<\/p>\n<h2>The question to ask is not \u201cWhich lens is best?\u201d<\/h2>\n<p>The more useful question is:<\/p>\n<p>\u201cWhich lens gives me the best balance between the vision I want and the optical compromises I am
